MHS sports physicals for upcoming year available in May

Maryville High School and Middle School athletes have an opportunity to get their physicals for the 2013-14 school year taken care of during the month of May at St. Francis Orthopedic Sports Medicine.

High school students whose last names start with A through I have first availability May 13 from 3-8 p.m.

Last names J through R are scheduled for May 15 from 3-8 p.m. Last names S through Z can go May 16 from 3-8 p.m.

Middle schoolers with a last name starting with A through I are scheduled from 3-8 p.m. on May 17. Last names J through R have the 22nd from 3-8 p.m. Last names S through Z are allotted May 23 from 3-8 p.m.

Cost is $20 per person. Cash or check may be made payable to St. Francis Family Health Care.

Please bring a completed and signed copy of the physical form to the clinic on the date of your physical time. No physicals can be performed if the parent/guardian has not signed the physical form.

Physical forms are available in the middle school and high school office or on the school's website at www.maryville.k12.mo.us.
